Establishing Care

We are honored you have chosen Chesterfield Pediatrics to provide care for your child. There are a few things to keep in mind when preparing for the first visit.

Making Your Child’s First Appointment

In order to provide the best care for your child, we require that all new patients have their medical records forwarded to us prior to scheduling. This allows us to familiarize our providers with your child’s unique needs, block enough time for your child’s first appointment, and ensure there is continuity of care during the transition. If we do not have your child’s records at the time of the first appointment, you may be asked to reschedule.

Selecting us as Your Child’s PCP

If your insurance requires that you choose a primary care provider, please call your insurance company to choose one of our providers as your child’s PCP prior to the first visit. If our front office staff completes this step at the time of your first visit, it may significantly delay your appointment time. Choosing a specific PCP does not limit you from seeing other providers within our practice.

The First Visit

Minors must be accompanied by a legal guardian at the first visit. Before your child’s new patient visit, we’ll ask you to complete some initial forms if you were not able to download them from our website or the patient portal and complete them prior to your arrival.

To make sure your family’s first experience with our office is a positive one, please arrive 15 minutes prior to your scheduled appointment time to ensure your registration is complete before meeting with your child’s new provider.

Guarantors

The legal guardian accompanying a patient to the first visit will be responsible for signing our financial policy as the guarantor. Payment is expected at the time of service, and in the case of estranged or divorced parents, the parent accompanying the child to the visit is responsible for any co-payments, co-insurance, or self-pay balances due at the time of the appointment.

Patient statements will only be sent to the address of the guarantor that signed and agreed to our financial policy. Chesterfield pediatrics will not intervene in any financial responsibility disputes between parents or other parties, and we will not look to anyone other than the guarantor to fulfill financial responsibility.

Custody Orders

We must have your child’s most current custody order on file. As advocates for our young patients, Chesterfield Pediatrics will not intervene in any custody disputes. Unless we have a court order on file stating that parental rights have been terminated, we cannot keep either parent from accessing their child’s medical information.

Late Arrivals

In an effort to respect your time, our staff strives to stay on schedule so that we do not run behind. Every effort will be made to accommodate late arrivals, however, please be aware that longer wait times may apply. Due to the comprehensive nature of behavioral visits, patients who arrive more than 15 minutes late for these appointments will need to be rescheduled.

  • Office co-pay
  • Custody order (if applicable)
